In The Netherlands as well. Here you have to fill out a short survey with questions if player was injured, could he continue and as last Q describe the situation. Works well and much quicker than writing it all down.
I recently talked with some Asian and Australian referees. They just say to the captains: you are heads, you are tails. Their idea is that you can show you take control there. What are your thoughts on this?
Do you use all the abbreviations in your reports or write them out there? In The Netherlands we don't use a lot of abbreviations, maybe just he assessors using H (home team) or A (away team).

A player has to leave the field of play after injury treatment. He may only re-enter after play has restarted, is what the Laws of the Game say now. But may he be substituted? Or does the substitution have to wait because re-entering was not allowed yet?
Thanks all for the comments. I think indeed it will lead to difficult situations when a goalie is already on a yellow. But at a shootout with one keeper already on yellow you can't treat them in a different way if they come off the line.
One of the biggest discussions in refereeing world was the penalty that Bjorn Kuipers awarded in the game between Croatia and Italy. Was it a penalty? And if so, why no retake?

In The Netherlands the fitness tests are only mandatory for national list amateur referees plus the referees of the two professional leagues.
No test for other amateur referees, although I think it would be good for every ref to have a certain level op physical ability.
