I like to listen to the pre-match briefing, as that usually includes the issue of senior/bench side (and often some interesting discussion😁)

The order on MOAS means nothing in terms of seniority, it's just a listing of officials. Referee establishes who will be bench side.

IMHO, red cards can be issued in all 3 cases. The injured player can stay on the f.o.p. to take the penalty.
