Is it offensive? Yes tick so you are fully justified to show him the red if by law. Some might disagree here but the offensive interpretation can vary between refs, so if you deemed it offensive at the time he walks. But yes I'd do the same thing as well.

On that one, seems a far fetch based on what is being circulated in the Saudi/arabic newspapers. I don't remember where I read this (or heard it) but it might help to think of it in a similar fashion to how you are looking at recognising SFP challenges. Is the arm extended or not? Jumping/leading in with force. etc might be some useful pointers in recognising the severity of the challenge...
