The LOTG does not require the keeper to be on the ground for the Pen. He can be jumping, just not unsporting. Hitting the bar is intended to put the kicker off. USB. Manage before the kick and no card needed.
I had a HB incident in an open mens game. Ball enters box from corner. Lots of scrambling by both teams. Defender gets nudged in back by team mate (unintentionally) and goes to ground. Puts his arms out to stop face plant. Ball rolls to one of his arms and stops dead. He jumps to his feet...
If both teams have a minimum of 7 players in their own half and the remaining players are not in the opposition half, get on with it. I've done it and it really starts the game with a rush. Time wasting fail.
I did an AA women's match a few years back. The CAR became vocal with my decisions so I dismissed him and put my flag away. Gave went much smoother after that. Would rather have no AR than one that causes issues especially when I'm the only one who really does not have an agenda.

Don't ask for another game. Step up to game as if last weekend didn't happen. Judge every game on its own merits, not last weekends.
Re the manager, I think he needed to be shown the error of his ways. Inciting players throws fuel on the fire which can cause the game to go pear shaped...
For me there is no time limit on how long he can play without a boot after it coming off occidentally, although he should be putting it on fairly quickly. However, as soon as there is another player is nearby or challenging, stop play and IDK against him for dangerous play.
