Confirming numbers for penalties

Cup game - 120th minute - player injured - needs attention on / off the pitch ...

Physio trying to patch him up on the sideline as the game restarts ...

Time is now up and the player is not back on the pitch yet ...

Does he need to be back on the pitch before the final whistle for him to take part in the ensuing kicks from the penalty mark ..?

No, because he is still in the 11 players that are to take a penalty because he hasn't been sent off or substituted. If you think logically he is off most likely because he has been fouled therefore you would be punishing him by not letting him take part in the penalty shoot out
 
Tho', arguably, the 'punishing' of players already occurs where a team of 11 against 10 or less have to reduce the number of penalty takers in their team

It is punishment on the players but not on their team because they get to use one less rubbish penalty taker

If he wanted to stay off (perhaps because his dream was actually a Chris Waddle, Stuart Pearce, etc. type nightmare) - to miss the penalties - this is ok too ..?

No, if he doesn't want to take a penalty then put him at number 11, if he doesn't want to take one a all, what is he doing playing football?????
 
He is still one of the eleve finishing the game, so he is one of the eleven who could take a penalty. He cannot opt out, just as any of the other ten cannot opt out.
