Say you referee a match (reds vs blues) and you show a number of cards. How do you tally them up?

You caution a Red player for unsporting behaviour. You then Dismiss the same player for a second yellow card, when he/she shows dissent later on in the game.

Is that 2 cautions & 1 red? 1 caution & 1 red? 0 cautions & 1 red?

The other two would suggest a direct red
 
Why do you need to tally them up? Is there a competition rule requirement?

I have never had to tally up my sanctions. But if I had to that would be 2 yellows and a red for me.
 
The second yellow card doesn't count towards the cumulative total in regards to a suspension for 5 yellows by a certain point of the season.
 
Looking at this screenshot of a football website it suggests:

1 yellow card & 1 red card

The red card being a second cautionable offence...

Typically card count for accumulation for 2Y->R is 0Y1R.
1Y+1R is 1Y1R.

Here in Canada, we have "caution summaries" upon which we note all cautions in a game _unless_ a player received two cautions (and thus was sent off). In that case, the cautions are summarized on the sending off report (aka "dismissal report").
