I always tell my captains to stop players surrounding me at kick off, and I will tell them to get their players away etc and will caution if surrounded and report etc, but it's just never occurred to me in a match to implement...

    Agree, but then if we know an injury is genuine and are sympathetic with it, we have the ability to manipulate things somewhat and get them on sooner compared with if it isn't genuine. If it's a blanket 'until the next stoppage' then we would lose that.

    Not sure this is feasible. If there is a genuine stoppage for a bad injury (maybe 10 mins plus) and then a team want to make a change during the added time which is effectively after only 80 mins of the match and we tell them they can't?

    Any time an offence is committed without the ball in play, it can not affect the restart, regardless of what that offence is. The only slight difference to this would be obviously an incorrectly taken throw in, but that isn't an offence as such.
