I present you the local rag as Sky Sports don't have the incident in their highlights, but essentially, Derby player gets a YC at the very start of the second half.

I reckon its for Entering the FOP without permission, there's obviously more info in the link.

If that was the reason for the caution, then the officials were wrong in Law:
If a substitution is made during the half-time interval or before extra time,
the procedure must be completed before the match restarts. If the referee is
not informed, the named substitute may continue to play, no disciplinary
action is taken and the matter is reported to the appropriate authorities.
 
The law as quoted relates to a sub being made and no one told about it.

Pure speculation but maybe, in this case, officiating team were told - likely at this level, surely, and for whatever reason player was told by 4th to wait a second or two before entering fop, but player ignored the instruction & entered anyway - THEN there would be justification in law for the caution wouldn't there?
