Detailed match cards

I posted a while ago with regard to going to OA games. Currently I referee youth games and I'm only required to note the number of goals scored and yellow and red card offences and their times.

However, with OA I need to take note of goal scorers and times and subs coming on and off (not rolling) and the cards I give out and to who. Does anyone know of a good match card that let's me easily record this data?

@micky2001 I can definitely say they are very useful for everything you describe as i was sceptical at first but now I wont go back to my old recording method.
