I believe they can be bought and sewn on by any of the big kit companies (I know A+H definitely do it, the RA shop too) - not sure if they can be purchased individually, however.

Yes, and county badges can be bought directly from the CFAs.

A&H sell badges from the FA badges the cup final referees wear at wembley, FAMOA, County badges, EA Sports badges, National League, Football League and premier league badges. Individual and already done.
