Match postponement letter

Any templates for letters to send to league secretaries to explain why a match has been put off?

You have to submit it in writing?

Basically state that you had to postpone due to (weather/pitch/other reason) because the (pitch/ground/other) would (endanger/make the game a farce/other).

Not sure if there is another way of putting it?

I have only ever explained my reasons to Captains and Managers then rung the fixtures secretary to inform him. Never had to put it writing. Are the club disputing your decision or something?
