Regarding attacking encroachment at a penalty, it 'should' be a retake if it goes straight in and an indirect free-kick if penalty is saved (and no defender encroaching.)
I remember Mike Dean making Frank Lampard take a penalty three times at West Ham but ti's true it's very rarely penalised.
VAR would not have intervened if the penalty had gone straight in but if an attacking player who had encroached had scored a rebound it would have (would have probably been a retake in this scenario as there were also defenders encroaching.)
