I think a lot of refs make everything ceremonial when its anywhere near the penalty area.

I always ask if they want the wall or not - which gets somewhat bemused looks. But in both my last fixtures players were holding up defensive freekicks waiting for the whistle. I guess I need to shout 'when you're ready' for those going forward.
