The only consideration should be whether a serious injury has occurred and which combination of players are injuredYou have to be quick on these decisions, i can understand why he didn't blow immediately as the ball fell to the attacker and had a chance of a shot to score, once the attacker played that ball backwards outside the box that should have been the time to blow.

Unofficially, two players flat out on the edge of the goal area is an instant stoppage and I can't conceive of any guidance out there to the contrary

Yeah fundamentally disagree with you there"head injury" means nothing to me. Even kids know to fake it.
